$30 Referral fee due at time of application (Cash only)

Information required to qualify for Dental Referral:

– Picture ID
– 2016 Tax Return, W2 or 1099
– 2017 Year to date earning for entire household
– Food Stamps, Alimony or Child Support Statement

Bring all medications with you when you apply for referral, this is for extractions and fillings only. This does not cover any type of dental appliances (Dentures, Braces, Crowns or Implants)

We also perform routine blood work at a reduced cost with a Doctors order.
Self pay only at time of service, we do not file insurance.
